This is a screenshot of a project I worked on in early 2003. The slider bars are dynamically generated from select boxes, as are the submit buttons. This means that in DOM browsers such as IE6 and Mozilla you get the nice interface elements, while in older browsers you still have accessible forms for meaningful data submission.
